Perry Cook Memorial Public Library offers free delivery of materials to persons of any age who are residents living within the boundaries of the Northmor School District, if they meet the following criteria: either temporarily or permanently confined to their home or care facility due to illness, injury or disability.

To receive home delivery, please fill out a Homebound Delivery Service application form and return to the library. Homebound Delivery Service Application forms are available for pickup at the circulation desk.
